SOCIAL ADULT DAY CARE SERVICES PROGRAM

What is a Social Adult Day Care Services Program (SADC)?

SADC is a structured, comprehensive program that provides functionally impaired individuals with socialization, supervision and monitoring, personal care, and nutrition in a protective congregate setting. The program also provides a variety of other services and supports, such as transportation, information and assistance, and caregiver assistance. In addition to addressing the individual’s needs for assistance in activities of daily living the program provides a secure environment and therapeutic activities aimed at helping participants to achieve optimal physical and mental/cognitive functioning. The program improves the quality of life for older adults by reducing social isolation and increasing social and community engagement. These programs are an important component of the community-based long-term services and supports system that delay or prevent nursing home placement. Many caregivers can continue to work and feel secure knowing that the social adult day care program will meet the care needs of their loved ones in a safe, nurturing, and stimulating environment.

Who Can Enroll in Managed Long-Term Care (MLTC)?

To enroll in an MLTC plan in New York State, an individual must meet all of the following requirements:

  1. Age Requirement
    • Be 21 years of age or older.
  2. Medicaid Eligibility
    • Be eligible for New York State Medicaid.
    • Some MLTC plans may also cover people with both Medicaid and Medicare (dual eligible).
  3. Chronic Illness or Disability
    • Have a condition or disability that is expected to last more than 120 days.
  4. Need for Community-Based Long-Term Care Services
    • Must require at least one or more of the following services for more than 120 days:
      • Nursing services at home
      • Therapies (physical, occupational, speech)
      • Home health aide or personal care services
      • Adult day health care (medical model)
      • Social Adult Day Care (under an MLTC plan contract)
      • Private duty nursing
      • Medical supplies, equipment, or transportation to health services
  5. Living Situation
    • Must be able to live safely in the community at the time of enrollment.
